The Patient Monitoring System, which patients use to take their physical readings, is connected to the phone. Through a number of peripheral devices connected to the monitoring unit, the patient can take their pulse, blood pressure, heart beat, weight and blood glucose readings. This data is then transmitted via a secure Internet connection to a secure data store.
Using a secure login, clinicians will be able to analyse the data that is presented in a graphical format. Clinicians will be able to drill down into the data to see readings at a particular point in time. The access to longitudinal data sets will enable medical staff to observe deteriorating trends earlier and initiate medical interventions sooner to avoid the patient having a chronic recurrent and being re-admitted to hospital. The system also enables clinicians to set thresholds for particular parameters and be alerted by SMS, email or fax so that as soon as adverse trends occur they can intervene.
MVC
The mobile video conferencing video stream is transmitted via a wireless link to a secure content server.
The content can be viewed by remote clinicians in real time using a standard broadband connection. Alternatively, the video stream can be stored and replayed at a later date depending on the situation. In addition, the clinician will be able to take digital "snapshots" from the video stream for closer examination.
