Connecting clients to care Project (CC2C)

The LMHA Connecting Clients to Care project was launched in November 2006 and is set to revolutionise patient care.

New technology will enable patients with chronic heart failure and chronic obstructure pulmonary disease to be monitored and cared for at home.

Remote patient monitoring (RPM) and Mobile Video Conference (MVC) technology will provide a practical solution to help reduce the number of presentations to emergency departments in public hospitals and provide real time support for staff working remotely in community health programs.

The CC2C project will deploy two new services across the Loddon Mallee Health Alliance broadband network.

 

Remote Patient Monitoring

Remote patient monitoring (RPM) technology will enable patients with chronic heart failure, chronic obstrucutre pulmonary disease, asthma and diabetes to be monitored and cared for at home. Patients will be able to take vital sign readings such as blood pressure, heart beat, weight, pulse and glucose levels in the comfort of their homes, and electronically transmit this information to a data storage repository. Care coordinators and specialists will be able to access the patient data for analysis and prescription of treatment regimes to be implemented by the patient in consultation with the health practitioner.

 

Mobile Video Communication

Mobile VIdeo Communication (MVC) wireless devices will support mobile health workers including district nurses and  community health practioners.  Using laptops fitted with a wireless received and a digital video camera fitted with a wireless receiver and a digital video camera fitted with a wireless transmitter, health practitioners working remotely in the community will be able to have "virtual consultations" with colleagues, doctors and specialists to support their clients.
